What’s a Smart Form?
In the most basic sense, smart forms are dynamic. Instead of static words on a screen, smart forms can change based on how a user answers a question using conditional logic. Want to segment questions to different types of users? Perform complex calculations? Smart forms get the job done.
Smart forms function according to if-then conditions. For instance, if a user is paying for a subscription, then they will be asked about their usage; if a user is not paying for a subscription, then they will be asked if they want to sign up for one.
Here are some use case scenarios:
- Sending emails to different addresses according to user answers
- Showing additional questions when a user chooses a specific answer
- Skipping pages in a multipage survey depending on user answers
- Offering different PDF downloads depending on the user’s choice
- Showing a different Thank-You message based on user answers to a quiz
- Password protecting a form
- Not showing the submit button until the user makes a particular choice

Just follow the “Conditional Logic Wizard” to smarten up your form.
Access the Conditional Logic Wizard
Go to the Settings tab in the Form Builder, then click Conditions on the left.
Select the condition you need.
What Each Conditional Logic Option Does
Show/Hide Field
The Show/Hide Field shows or hides a field when the specified condition is met.
For example, if a user answers Did you use our product? with “Yes”, then the form will show How would you rate it?.

Update/Calculate Field
You can create conditional logic rules to perform calculations.
Let’s say you want to add values from two different fields and have the solution appear in a text box labeled “Total”. You can specify the fields to be added and when they should be added.

Enable/Require Field
This option allows you to set a form field as required or not required depending on the user’s input.
You can also enable or disable form fields. This option can be used to conditionally set or change a field’s Content Mask. The Content Mask helps users enter data by ensuring a predefined format, such as an international phone number, will be followed.

Skip To/Hide a Page
It is also called skip logic. It allows you to skip or hide pages based on a user’s answers.
You can ask additional questions on a new page or simply end the form.

Change “Thank You” Page
In this option, you can redirect to a different Thank You page according to the form submission.
Let’s say you list your white papers in a dropdown list and prompt the user to select one. After the form is submitted, the user will be redirected to the white paper they selected.
You can also display a customized Thank You page according to the form response. If your form is a quiz and the user’s responses for the quiz are not correct, it might be useful to provide the answers or additional resources on the next page.

Send different email alerts based on your users’ responses.

Save your interactive form after completing each condition, then run a test. Before adding more conditions, make sure the existing ones are functioning properly. Otherwise, your smart form might not be so smart after all.

I have conditional logic throughout the form and it seems to be setup correctly, but when I preview the form it only shows the header of the second page. It should go from first page to second page to thank you page. I have conditional logic setup that uses information from page 1 to show certain fields in page 2. Is that why it's breaking? Hi There, I'm trying to have it hide the next question in a questionnaire depending on the value that users select from a drop down in a grid. That grid has first a column of dropdowns and then to columns of text fields. When I say IF "that question" Contains Value "a specific value from the drop down list column (first column in the grid)" it doesn't do anything? But if I just type that value in next columns text box field, then it works? How do I get the value from the drop down in the grid be used in the condition?

Each petition form usually hold about 10 signature slots for the individual city or county. Is it possible for jotform to shoot out pdf form exactly like the paper form and the client can electronic sign the petition( ex. In slot 1 where print name and signature , as well as voter address. After a complete signature with print name and voter address. The form return back to me. And I would categorized the completed signatured , combine it on 1 sheet and print it on 1 sheet. I am setting up a jotform where a core group of 5 people will always need to receive a notification that a form was submitted which I believe I should set up under EMAILS under the SETTINGS tab.
In addition to that group of 5 people, there may be occasions when other departments, such at tech and communications, may need to receive notifications only if their services are needed.
My question is, do I set up notifications for the core 5 people under EMAILS under the SETTINGS tab, then in order for the other departments to receive notifications when needed, set up under the CONDITIONS tab a CONDITIONAL LOGIC so the notification emails goes their email? If so, do I need to put in that departments' email only or do I need to put in the core 5 people AND the email of that department?

I would like the thank you page to give the results of this test so say they got 80 out of 100 it would be 80% and i would like it to give a pass or fail based on this percentage.

I have a form with a multiple choice question that could be answered with multiple different combinations of answers. I need to use conditional formatting on it, but if/then isn't enough. The option that I am looking for is "unless" and I can't see a way to make that happen. If they pick "Option C", I want the form to skip the next two questions. If they pick "Option A" or "Option B" by themselves or in any combination with "Option C" I want the next two questions to show up. Any idea how to make that happen? I have already tried the "Is not equal to" option to see if that would work and it appears to give basically the same result as the "Is equal to" option when applied to the other choice.

(3) N/A

I have a form that previously worked fine with the conditional logic i input, but between disabling the form and re-enabling it, it now seems to not be working correctly even though none of the conditional logic has changed.
I had it set to hide an address field if a shippable item type was not selected, and to show if it was. however it's showing the address field for non-shippable products (I do artwork commissions and only ship certain types of physical artwork, the rest are digital only)

IF any of the drop downs are equal to billable,
THEN the box is automatically checked by the form.
Then say there is a mistake and clicked billable when they didn't mean to and someone needs to correct the form:
IF all of the drop downs are equal to Non-billable or blank.
THEN the box is automatically not checked by form.

I've tried searching this elsewhere but can't find it. I have 2 conditional logic questions, and they're working fine, but when I go to generate a pdf to print it, those 2 questions are printed at the very bottom of the form...which makes no sense.

For example, when someone fills out the form the 1st time I want them taken to a special offer page. I created a Jotform (https://www.jotform.com/build/81617014808152) that contains several questions (12A - 12G) in which I want the user to be able to "Mark All That Apply". When I preview the form and click on "Fill Form" button, ALL of the options for these questions are checked and I cannot uncheck them. How do I fix this problem?

I plan on the sheets app organizing the Jotform data automatically through triggers (onEdit) as it comes in, but that may not work if the Jotform API uses Google Spreadsheets API to edit the form directly. The example logics I've seen on this thread have a STATE of "is Equal to". I don't know what the functionality of the Contains state is in regards to the input table columns. As it stands right now simply putting the VALUE as the name of column of the input table, for which it sends the notification email when someone answers in that column, does not work.

Can you send the Jotform data to automatically update more than one Google Drive Spreadsheet?
I had a working google doc integration which broke (I think because I moved the Google Folder). I am trying to reintegrate and am now getting a failed integration message or it is telling me it is integrated to an undefined spreadsheet. Help!

My form is basic "are you attending" form.
Respondents can answer 'yes' or 'no'. I can stop after a number of submission but I would like to stop the form after X 'yes'.
Is there a way to do so ? Like if I have W submission with the answer 'Yes' to the question 4 do not show question 4 anymore?

I have integrated the form with google docs. However, if I add a column, future submissions are not posted to the right column. How can I have my submissions data map to the correct fields?

I am trying to create a form in which by using "the spinner" clients can select the quantity of the product to purchase. After that lets say for example they chose 1, then they will have the option to customize that item by using radio buttons. My problem is that the quantity can go from 1 item to 200 items, so I would like to automate the process of creating a new radio button for every item they choose to buy. Please help me to find a viable solution without the tedious work of creating individual logic configurations to each quantity.
We have been integrated with google spreadsheets for some time. The integration seems to have broken a bit in the past week however, as the form no longer passes the date of submission to our spreadsheet. All other data is still being uploaded to our sheet however. We just have a blank column for the dates of forms submitted in the past week or so. Please advise.

Can you have multiple "ifs"? For example:
You have four age brackets: 5-17, 18-25, 25-35, 36+
Can you say IF: age bracket is 5-17 OR 18-25, THEN: charge $20?
I just wanted to update my earlier problem. I was trying to use the conditional statements to update prices if a certain box was checked but couldn't figure out how to adjust it for all the different prices. Since last night I figured out that I had to make a statement for each price and it worked. Was a pain to write a statement for each one, hopefully it will work as intended.
I have made a form and let me say, this is amazing. It works so well. I love the ease of using the logic. I am utilizing the conditional logic that IF choice A, then SHOW option 1, IF choice B, then SHOW option 2.
It works great online, but when converting to a fillible PDF, the logic does not make the next question appear. I choose choice A, and nothing happens.
Is this not possible yet?

I am using skip logic on a question that is yes/no. If 'Yes' then display specific question and if 'No' then a different question comes up. No matter how I seem to set up the rules, either the 'Yes' follow up OR the 'No' follow up question populate on the unfilled form. Am I missing something that will not allow either alternative question to show up until a 'Yes' or a 'No' is selected?

my form has multiple options for people who are subscribing (this is a printed magazine), updating an address or cancelling a subscription. The conditions are working for jumping to a page of fields. That works, but I can't get it to skip pages to the end after this step. It used to work on FormsCentral.

First of all Thank you! This is fantastic. I just made my first form and converted it to fillable pdf. as instructed, but my images don't show on the pdf? I have two images on the form, one is my logo and the other one is a guide to help the customer answer a specific question. Is there a trick to this or is it not possible to include images in the pdf?
I have set up my form with page breaks between sections. Depending on someones selection, certain pages can be skipped, but I can't seem to skip the whole page and onto the next section without leaving the "back/next" bar. If I try to include it in the skip logic, when I hit next, it comes to a blank page. If I leave the "back/next" button in, then they keep coming up with no information behind them, and it looks tacky. Is there a way to hit "next" and skip a whole section?

I was trying to use conditional logic in my form and i am having trouble with it. So I used purchase order to display my products. What i want to do is, once the user choose his product, i want the form to automatically direct it to the end of the form. As i was not able to do that, i used page breaks split the form into two parts. However, the user still needs to go all the way down on the form to choose the next button to be redirected into the information section. IS there a way to shift to the 2nd page without using the next button using conditions?.Thank you
Is there a way to use the conditional logic feature to "auto fill" a persons name throughout a form? I have a long form that has the words "Bride" and "Groom" throughout it, in radio button fields and multiple choice fields, etc. I would like the Bride and Groom to fill in their names on the first page and for all of the other spots throughout the form that currently say "Bride" and "Groom" to automatically fill with the correct names. Is there a code to do this with?

My form is currently connect to a Google Doc folder, but I can't figure out where this folder exists. It is possible it is connected to someone who no longer works here. I am setting up a form for a bike a thon fundraiser where people can support a specific person. I have a dropdown box with names and I am trying to set up a condition using "is equal to" so when a person is sponsored they receive an email that they have been sponsored. I got one condition set up but when I tried to set it up for additional people it will only let me select the top name in the dropdown box.
How may I have the data that is filled through a particular form to go to multiple spreadsheets according to the input, or to different sheets in the same spreadsheet?
I am trying to set up conditional logic based on information supplied by my sales rep.
If there is one components then I need just one component section and hide the rest of the components. How can I do that??
It will hide everything if there is no selection, but if I select a component then it shows all 4 components.

I am trying to set up a date condition. If today's date (which is the first date field on the form) is after the 20th, then a different date field would pre populate with the first day of the next month, without being able to change it to a previous date. My form is and want cid_21 to be populated with the today's date or later if cid_5 is todays date is before the 20th of the month. If it's (cid_5)the 20th or later I want cid_21 to only allow the date the be the first of the next month or later.

However, when I go to authenticate my Google accounts (Drive and Spreadsheet)and Accept Jotform's request for access permissions, I'm greeted with a blank page. I've tried all my browsers (IE, Chrome, Firefox, Safari), both signed in and out of my Google accounts, but with the same result. Could you please help me discover what is going wrong, as it is enormously frustrating to have such a potentially-useful tool fall short at the last minute.

Is it possible to have a user fill out the form, then make a payment while in the form - and have the form determine if the paypal payment was successful, and then record all the data and the approval/failure of the payment into a google doc?

How can I do the following: Based upon a particular response I want go to one of 3 other forms. The conditional logic works fine for specifying different pages, but not for specifying different forms. Also, is there a way to add an invisible page break (one that doesn't have the previous-next buttons).

I am trying to collect visitor contact information with my jotform and transfer it to my Google contacts. Is there a way to get my jotform contact list into my Google contacts? Or, am I able to setup the Google docs spreadsheet created from my jotform to import cleanly into my Google contacts? I used the created Google docs spreadsheet and exported to a .csv file then imported to my Google contacts. It got the first & last names and emails in the correct fields but it dumped just about everything else into the "notes" field in my Google contacts?!?

Is there a way to add multiple "then"s or results to a condition? For example, for the MMiT Application form I'm working on, I want to hide all questions until they're answered one at a time. (Once the full name is filled in, then it will show 'Have you paid your dues?'; if they select 'yes', it will show 'Please list all available...' - if they select 'no' it will hide the rest of the form.
Is there a way to do this without going into each specific question and creating multiple conditions for it? I can see that you can add multiple 'rules', but not multiple 'then' sections.

Jotform is pretty neat. This is complementary to what we have done at delighta.com. You can enter data in your Google spreadsheet via SMS from as many mobile phones as you want. We believe we are the first company to successfully integrate SMS with Google spreadsheet and calendar.

One of the advantages of the google integration "workaround" you proposed in an earlier post (Jotform HTML report updated every n minutes) is that the first row of the spreadsheet is empty, the form header starts on row 2 and the spreadsheet fills from row 3 down. This very convenient as I can test for the number of entries in a column at the head of the column and use conditional formatting to indicate that I've exceeded capacity in a class.
Can the starting row of the Jotform Google Docs integrated report be specified to allow the same functionality without interfering with the subsequent automatic data entry?
Is there a way to name pages? Instead of trying to remember which is "page 1" and "page 4", etc, for a long list of pages, it would be easier if each page could be named; and those names used in branching conditions.

I have tried to apply the same conditions to two different buttons. When I set all the conditions to the first one, it works fine. But then when I apply them to another button, they stop working on the previous button.
Surely I must be able to make the conditions work all over the place. This is on formID=21704271832145
Also, is there some sort of settings I have to adjust to the Google spreadsheet. When I fill out the Jotform, the data is not pushing to the Google spreadsheet.
Can you move the folder that is created (i.e. I want to place the Jotform folder for registration for our golf outing into the Golf Outing google docs folder)? Or, does it have to remain outside any subfolder to map the data correctly?

This is halfway working for me. It only fills out some of the questions on my G Docs spreadsheet. The Jotform email I get has everything filled out but multiple fields don't go through to the spreadsheet.
Umm... can't authenticate for google docs. Click on the green Authenticate button and I get this:
Error: redirect_uri_mismatch
The redirect URI in the request: https://www.jotform.com/api/google/ did not match a registered redirect URI

Since integrating, I've added some new sections to my form, which are not showing up in my Google Doc. I'd rather not reintegrate because I spent a lot of time personalizing the Google Doc, so I'm wondering if there's something I need to do to for the new sections of the form to show up.

I am exploring the options of using jotform on a professional level and are testing out the service before I purchase a business account for 50 dollars a month.
I created the form ( very surprised how easy it was) and after the google docs integration (that went through as well) the responses dont get parsed to my google docs account. (only once!)
I tried linking it to a different google drive account but with both instances the result of the form only gets registered once.
Again, please let me know if I can help or I am doing something wrong!

I've connected my form to google. When I did it, the submissions I had appeared on my google spreadsheet but I'm not getting any of the new submissions on it. Did I do anything wrong?
i am testing this right now. i am editing a submission using the edit link sent from an autoemail email. but it seems that the edits of an already submitted are not reflected in gdocs. any steps i missed out? thanks

Can we customize this at all? I have an order tracking spreadsheet that I currently use. It uses a Google query in the URL to show only rows based on client information they submit. As orders come in through jotform I manually enter them into my Google spreadsheet. Users can then enter their account number and see all active orders they have submitted.
I would love to be able to customize the way Jotform integrates with the spreadsheet and even better, have it add to my current document.

When viewing the submissions in the google "drive" spreadsheet view, can you edit them and then see the changes reflected on the Jotform site when viewing there? Or is the information flow only one direction?

I'm getting an error message that says "Error: redirect_uri_mismatch" when I click the "Authenticate" button. I accessing my forms through the Jotformpro.com url.
Great! Though Google Docs is not with us anymore. Has become "Drive" now.
This is very cool, but it's not working for me.
I Click Authenticate and then Click the Blue Google Allow button.
Then it asks for Folder and Filename which I leave as default and then press next.
It sits there for 10-15 seconds then comes back with this error.
Great news, just one question tho.......
Can the submission still be e-mail to specific users aswell as adding to the google docs spreadsheet.

I set up registration using the Jotform and when the meeting is over I would like to set up a survey and email the survey out to the attendees of a meeting. I have one question on my form, "What is your primary business activity?" This has a list of 20 or so responses. I set up about 15 conditions from those answers that will direct the person to another question "Describe your business activity." I have previewed the form and when I cick on "Advertising" for example the question "Describe your business activity does not appear below it for some reason. Please tell me how I can fix this.

Most of my customers are going to my contact form from different pages. They choose one of these different pages because of the product choice they made. When they arrive to the contact form, they have to choose again in a list the product that were on the page they're coming from. You can not do it directly. As you said there is no option for this. It is in our future feature list. What you can do instead is:
Create a form and create the conditionals so that if a user chooses some of the fields he/she will be redirected to a different form upon submission.
You can change the redirected form addresses according to the selections of the user.
I am setting up a product order form.
1st half of my form contains: Name, Email, Address, Phone.
2nd half of form contains:
5 unique products each with a quantity selector.
I am hoping to add a conditional product for shipping wherein if the order total is below $25 then shipping is $2.95 or above a total of $25.00 shipping is free.
The Conditions Wizard is only showing me the first four options of name, email, address and phone and then the title for the 2nd half of my form. (those are my only show/hide field under the "if" option.
There is no option to apply conditions to any of the products of which shipping is one. How can I set this up?
I have a form that displays text when the user chooses an option. The only way I have found for this to work is to set up two conditions where the form HIDES the text if the condition is NOT met and then to DISPLAY the text when the condition IS met.
If the text could be set to hidden in properties and then a condition could DISPLAY it would be much more convenient.

Please ask your questions to the forum.
Start just with as if you are adding a condition to a textfield.
On the top toolbar Setup & Share> Conditions is the way for adding Conditions.
What you should enter to the area is the number of stars.(1,2,3...)
